How to Apply
Intending applicants should complete an Application Form:
- DHB applicants must have their application approved by their manager and endorsed by their CEO
- Health sector applicants who are not employed in a DHB need the support of their own organisation and the endorsement of their National Health Agency CEO
- You may download the Application Form.pdf (55 KB) or obtain one from your CEOs office
- Non-health sector applicants should contact the LAMP administrator
