National Standardised Forms and Templates
A national standardised set of forms and templates for use with the various services is provided as a minimum requirement to providing the service.
These provide for a nationally consistent data set and include the consent form, information gathering, outcomes assessment and reporting forms.
If a service provider wishes to add to this data set they may, but the standardised forms represent the minimum allowable data collection.
