An amendment has been made to the list of CVD and Diabetes basic phase indicators that were originally proposed to the Programme’s Governance Group. Issues raised by Shelley Frost at the Governance Group Meeting on 25th July on behalf of her constituents relating to the CVD and Diabetes Basic Phase Indicator set were discussed at the Advisory Group Meeting on the 5th August. Professor Les Toop also attended the Advisory Group meeting to discuss these issues.
As a result of this discussion, the Advisory Group provided a paper to the Governance Group (via the Chair) recommending an amendment to their original advice on the CVD and Diabetes Basic Phase Indicator set. The amendment is that the CVD Risk Assessment >=15% indicator be excluded and further analysis will be performed to determine whether the 15% value is appropriate.
Therefore if the amendment is accepted by the Governance Group on the 12th September the confirmed CVD and Diabetes basic phase indicators that the Programme will be implementing are:
Ischaemic CVD Detection
The number of people enrolled in the PHO who have had an ischaemic CVD event or diagnosis of CVD event as per the CVD definition and have this coded appropriately in the practice PMS system.
CVD Risk Assessment
The number of people enrolled in the PHO that are eligible (as defined) for CVDRA who have had a valid CVD risk recorded within the last 5 years.
Diabetes Detection
The number of the PHO enrolled population that would be expected to have been diagnosed with diabetes and have this coded appropriately in the practice PMS system.
Diabetes Follow-Up
The number of the PHO enrolled population that have been diagnosed with diabetes that have had a diabetes annual review (Get Checked or equivalent) in the reporting period.
The Programme expects to start measuring the new indicators in January 2009 (which is when the 1 July 2008 period is due to be reported). Baseline information will be provided on receipt of a complete set of Clinical Performance Indicator (CPI) reports.
The CPI report, which contains PHO aggregate clinical query data, has been endorsed by the Programme Governance Group for inclusion in the PHO Service Agreement. The Programme expects the first set of reports from PHOs to be provided late October/early November 2008. Targets and weightings are still to be confirmed.

The Programme’s Governance Group has endorsed the Programme’s proposal to increase the provision of data to PHOs. This should enable PHOs to introduce improvements to their organisations within the current performance period rather than 3 months after the period being measured (when Programme reports are released). The Programme is currently documenting requirements and the specifics of the monthly extracts will be communicated to you in due course.

The Programme conducted analysis to ascertain whether the current target setting model continues to be as reasonable and realistic as it was when it was first introduced on 1 January 2006. The paper detailing the findings of the analysis that was tabled for discussion at the Programme’s Governance Group meeting on 25 July was deferred and has now been tabled for discussion at the meeting being held on 12 September 2008. Information relating to revised target setting processes will be communicated in due course, once endorsement from the Governance Group has been received.

The Programme held its bi-monthly meeting on 5 August 2008. 15 out of 21 members attended. Key topics discussed were
Professor Les Toop attended this meeting to present his concerns regarding the proposed CVD and Diabetes basic phase indicators to the Advisory Group.
Ashley Bloomfield from the Ministry’s Tobacco Control Project also attended this meeting to provide an update on the work which the subcommittee is undertaking. The framework for the development of smoking cessation indicators is being guided by the ABC approach (Ask, Brief Advice and Cessation advice offered).

Please be reminded that the Programme measures 4 of its indicators on an annual basis. The annual indicators are:
For the Influenza indicators (while still considered to be annual indicators) targets are set 6 monthly and payments are made 6 monthly based on actual performance. It is important to note that the Programme now uses the flu season pertaining to the period being measured to assess PHO performance.
For the Referred Services indicators, targets are set annually and half of the expected annual payment is made on completion of the first 6 month period (an ‘upfront’ payment) while the remaining balance is paid on completion of the second 6 month period (a ‘wash up’ payment based on overall actual annual performance.

Please take note of the following minimum payment thresholds that must be met before a performance payment will be made by the Programme:
Threshold minimum of 40% required at the end of an annual performance in order to achieve payment
Threshold minimum of 50% required in order to achieve payment at the end of each 6 month performance period
Threshold minimum of 1 required in order to achieve payment at the end of each 6 month performance period
These thresholds apply regardless of whether a target is met or not.
