ADVISORY GROUP MEETING
The first meeting of the Programme’s new Advisory Group membership was held on 7 April 2009 in Wellington, with 11 out of 14 members attending.
The Group discussed a range of topics, including the:
Details of the key messages resulting from the meeting will be posted on the DHBNZ website once the Governance Group has reviewed the recommendations on 29 May.
The next Advisory Group meeting is scheduled for 8 June 2009.
GOVERNANCE GROUP MEETING
Here is a copy of the media release made by the Governance Group after its meeting on 20 February 2009. Please note that at the time the statement was released, the next meeting was due to take place on 8 May. This date has now been changed to 29 May 2009.
The PHO Performance Programme Governance Group held another successful meeting last Friday 20 February in Wellington.
Strategic Direction
Positive feedback was received on the draft set of Strategic Principles for the Programme. Addition of a purpose statement, and a number of other improvements were agreed. Revised principles will come to stakeholders for any further comment during March and will then be developed into the draft Strategic Plan.
Reducing Inequalities
The Governance Group had a full discussion on Reducing Inequalities and concerns raised by the Maori PHO Coalition. The Coalition sees the programme as fundamental to efforts to reduce inequalities, and wants to ensure that the programme is successful. The Governance Group Terms of Reference requires all members to demonstrate population health and health inequalities perspectives. Health Gain and Reducing Inequalities is at the core of the purpose of the Programme. All programme activity must drive to this objective - the Programme's attention to reducing inequalities will be measured by its actions. Reducing inequalities is included in the Strategic Principles currently under discussion. The methodology used by the Programme to establish incentives and to evaluate indicators includes specific attention to inequalities and high need, and this will be reviewed to ensure that it is effective. A specific review of how the programme approaches reducing inequalities will be completed.
Advisory Group
Remaining gaps in the Advisory Group are being filled. The Governance Group will sign off on the final membership to ensure an effective and appropriate mix of expertise and knowledge. The first meeting of the refreshed group will occur in March 2009
Roadshows
Serena Curtis-Lemuelu and her team are completing a series of roadshows around the country. The roadshows build on previous events and aim to ensure that Provider, PHO and DHB staff are fully briefed, and have their questions answered.
Data Security
Further work has been done to clearly describe all data flows in the programme and the security arrangements around those flows. The Business Process documentation contains this information and has been shared with the Governance Group.
A high level summary of data flow and security requirements is being prepared for signoff by the Governance Group at their next meeting. The summary will then be provided to the wider sector.
Indicator Evaluation Process
The methodology for selecting and evaluating indicators was discussed and a number of improvements agreed. A refined Indicator Evaluation Process will come to the next meeting for signoff.
Sustainability Payment
A sustainability payment has been proposed to assist development of infrastructure within PHOs and the PHO-Provider relationship. The funding comes from the total incentive pool funding provided by the Government. The Governance Group agreed that the Programme had a key role to play in assisting development of infrastructure, but did not want to simply spread the funding across all PHOs. A more targeted approach was sought to ensure value for money from the funding. The Governance Group has asked for further advice on this and will seek further input from the sector.
DHB Reporting
DHB level reporting was supported, provided that DHBs discussed material and interpretation with their PHOs before release. Several PHO Groups are moving of their own accord to release information at the PHO level.
Ministerial Committee
The Minister has established a Committee to review expenditure to improve spending quality and services to patients. The Committee has asked for information on PHO Performance Programme given that the Ministry funds the incentive pool ad part funds the administration. The Governance Group agreed to submit a package of material to inform the Committee of the Programme covering activity, pros and cons, direction, opportunities and threats. The Governance Group would also seek to meet with the Committee.
Next Meeting
The next meeting of the Governance Group is on Friday 8th May, with Agenda and papers provided to the sector Tuesday 14 April.

BULLETINS
The Programme has decided to issue its Bulletin updates every two months from now, instead of monthly. This gives the Programme time to gather enough information to be useful to you in an update, The Bulletins will continue to be posted to the public pages of the DHBNZ website on the first day of the month after the report is due.

PROVIDER LIST TEMPLATES
There’s been a delay with the Programme forwarding the next series of Provider Lists for the period 1 January to 31 March 2009 due to some PHOs being unable to confirm their Provider Lists for the period 1 October to 31 December 2008 within the required timeframe.
The Programme will be contacting PHOs directly in coming weeks and forwarding the templates for completion. The Programme would be grateful if you could complete and return these as soon as possible.

QUARTERLY REPORTS
Programme performance reports and scorecards (covering the performance period 1 July to 31 December 2008) have been posted on the DHBNZ website.
The purpose of the:
Payments relating to this performance period will be processed for payment to PHOs on 15 May 2009. Flat file data will be sent out to PHOs shortly.

WORK IN DEVELOPMENT.....
CHANGE REQUEST PROCESS
The Programme has developed a process to better manage the way changes are proposed to the PHO Performance Programme. As interest in the Programme grows, the Programme recognises that good methods to identify, document, develop, approve and implement changes are required.
The process (which has been endorsed by the Programme’s Governance Group) consists of a number of phases which, depending on the type and size of the change requested, require sign off from either the Governance Group or DHBNZ CEO. The phases are:
This phase identifies at a high level what activity the Programme team is being asked to do, affected parties, the justifications for having the Programme team perform the activity and whether the activity is supported by the Governance Group or DHBNZ CEO.
This phase identifies the best way for the Programme team to do the work requested, after consideration of all the ways in which the work could be done. Consultation with the sector is at its highest during this phase.
This phase identifies in more detail what needs to be done by the Programme team and stakeholders to implement the requested work. This phase focuses heavily on scoping and building the solution.
This phase identifies whether the technical requirements developed for the solution will work as intended. What has to be tested, the environment in which this will occur and how it will be done are addressed in this phase. Expected functionality will be proved or disproved.
This phase sees the Programme team release the requirements into the ‘live’ production environment. The outputs from this phase are the deliverables described in the Requirements Document (as part of the Development Phase) and in the details of the Project Plan.
This phase looks at how the Programme team addressed each phase of the activity requested. Identifying any lessons learnt is an important part of this phase so the Programme can improve of the way it works.
This phase reviews the impact of the change on stakeholders, the environment and Programme objectives, and asks if the benefits/advantages envisaged have been realised. This is very much the post implementation reviews phase (occurring at least six months after the change has been implemented) and will be performed at the discretion of the Programme Manager. Where it is found that:
(a) The change has achieved what was intended – the Programme will report this successful result to the Governance Group and DHBNZ CEO;
(b) Implementing the change has had a negative or little impact – the Programme may perform a second evaluation at a later stage to confirm if the decline is continuing or reverse the change (in which case a new request for work will be required).

INCREASING THE FREQUENCY OF DATA TO PHOs
Monthly Reports for PHOs are now available via the secure pages of the DHBNZ website. These give each PHO a snapshot of their performance against selected programme indicators for the month being reported, using the most up to date information that’s available.
The reports provide coverage information on cervical screening and influenza vaccinations so PHOs can compare their current progress towards targets with their actual performance for the same period of the previous year. A graphical representation of the information is also available.
The Programme is continuing to work with the Ministry of Health to get timelier breast screening data and the Programme will let you know when that information is available.

RELEASING PATIENT IDENTIFIABLE SCREENING INFORMATION TO PHOS
The Programme continues to work with the National Screening Unit’s BreastScreen Aotearoa and Cervical Screening Programmes to identify ways to allay any concerns from the public about patient NHI identifiers being released to PHOs. The Programme will update you further as this work develops.

SMOKING INDICATORS
At the request of the Governance Group the four smoking indicators developed for Programme implementation by the previous Advisory Group members (in consultation with the Ministry of Health) have been reviewed by the new Advisory Group.
The Advisory Group has made recommendations to the Governance Group which will be discussed at their next meeting on 29 May. Further updates will be provided in the next programme bulletin.

While the Governance Group has agreed that the Programme has a key role to play in assisting with the development of PHO infrastructure, a more targeted approach is being sought to ensure value for money from the funding. ADVISORY GROUP MEMBERS APPLICATION PROCESS
The Programme has sought additional members for the Advisory Group to ensure it has the right membership mix. Membership applications will be considered by the Governance Group on 29 May 2009. Applicants will then be advised of the outcome and an announcement of any additional members will be made following that.
