A new Programme indicator set has been endorsed by the Programme’s Governance Group for implementation from 1 July 2008. This new indicator set will be reported by the Programme for the first time in January 2009.
There are four new CVD and Diabetes basic phase indicators that have been introduced with funding. These indicators will be measured against both total and high need populations:
1. Ischaemic CVD Detection
The number of enrolled people in the PHO with ischaemic CVD coded at the end of the reporting period
2. CVD Risk Assessment
The number of enrolled people in the PHO within the eligible population who have had a CVD risk recorded within the last five years
3. Diabetes Detection
The number of enrolled people in the PHO diagnosed with diabetes
4. Diabetes Follow-Up
The number of enrolled people in the PHO with a record of a diabetes annual review during the reporting period
For the 1st 6 month implementation period (i.e. 1 July to 31 December 08) 5% minimum payment thresholds will apply as per the table below (while the Programme continues to work with its Advisory and Governance Groups to determine targets).

The Programme expects to have Governance Group endorsed target bands in place to measure the 2nd 6 month performance period (i.e. 1 January to 30 June 2009).
Baseline information for the above indicators will be provided on receipt of a complete set of Clinical Performance Indicator reports (CPI reports). Please note that most PMS and PHO vendors undertook work early last year to prepare PMS for the extraction of CPI reporting data to PHOs. Please be assured that the current CPI reporting functionality that has been implemented by vendors meets the needs of the Programme. If you have any questions regarding the technical requirements of the CPI report please contact your PHO IT vendor. Future changes to the CPI report are presently being consulted with PMS vendors.
For those PHOs that are ready to submit their reports to the Programme for the 1 July to 30 September 2008 quarter – you are invited to do so (noting that the Programme will require both this quarter and the 1 October to 31 December quarter data to be submitted by 20th January 2009). Reports can be emailed directly to the Programme Manager (serena@dhbnz.org.nz).

Target bands and Programme goals have also been adjusted for some Programme indicators to reflect current times. An updated version of the ‘National Indicator Set and Weightings’ document (version 6.0) will be tabled with PSAAP for inclusion in the next variation of the PHO Agreement. Changes to the payment amount per enrolled patient will also be submitted to PSAAP once the amended amount has been confirmed.

The Programme’s Governance Group has endorsed the Programme’s proposal to increase the provision of data to PHOs. The Programme has documented requirements which have been tabled for discussion at the Programme’s next Advisory Group meeting on the 14th October. The specifics of the monthly extracts will be communicated to you in due course.

The Programme conducted analysis to ascertain whether the current target setting model continues to be as reasonable and realistic as it was when it was first introduced on 1 January 2006. New target bands have been endorsed by the Programme’s Governance Group for implementation with the 1 July 2008 indicator set for the following indicators:

The PHO Performance Programme (on behalf of the Programme’s Governance Group) invites interested persons to submit an application to become a member of the PHO Performance Programme’s Advisory Group.

The third meeting of the PHO Performance Programme’s new Governance Group was held on 12 September. There was productive discussion about the importance of:
· a clear strategic direction for the Programme supported by all parties
· clarity on who needs to do what to deliver indicators
· links to other activity in the sector such as the Quality Framework and professional standards
The Governance group held a workshop to discuss the strategic direction of the Programme and agreed a number of principles. These included understanding the influence the Programme has – and can have – on health targets, and how it can provide a catalyst for change. Also of prime importance were access and security of information.
From this workshop it has been agreed that a range of areas need to be further explored to ensure that the future direction of the Programme is inclusive of the agreed principles and that the Programme is effectively linked with the overall direction of the PCHS and key providers within primary health care.
While this work is being undertaken the Governance Group accepted a new indicator set for implementation from 1 July 2008 to support the continued development of the Programme. The indicator set had been amended by the Advisory Group following advice from the sector.
The cardiac indicator for high risk individuals (previously CVD risk > 15%) was not introduced. A review of the cardiovascular guidelines by the NZ Guidelines Group will assist in better defining who the high risk individuals are. Further work will then need to be done before considering it for reincorporation into the Programme.
Also during this meeting, the Ministry of Health confirmed the availability of an additional $12 million to support the Programme.
