CVD and Diabetes Capability Assessment Plans
The Programme has completed its analysis which saw a comparison undertaken between the 2 year old immunisation data held in the National Immunisation Register (NIR) database and the 2 year old immunisation data supplied in the Service Utilisation (SU) reports provided by PHOs to the Programme.
To perform the analysis the Programme contacted all PHOs and asked for the following NIR data (noting the Programme does not currently have access to the NIR):
41 PHOs responded and a comparative analysis was undertaken for the January to March 2008 quarter only as many PHOs were unable to provide the Programme with NIR data prior to November 2006 because of the NIR ‘go live’ date.
In summary the Programme found that there was only a 4% difference (whereby 78% of immunisation data was available via the NIR as opposed to 74% being included in the SU reports) between the 2 data sets for the January to March 2008 quarter.
The Programme will be recommending to its Advisory Group (at the 5 August meeting) that the Programme should continue to source immunisation data from the SU reports based on the following rationale:

The CVD and Diabetes indicators developed for the Programme have been endorsed by the Programme’s Advisory Group and are now currently with the Programme’s Governance Group for endorsement.
The initial ‘basic phase’ indicators are listed below. Please note that the indicators are proposed and are subject to change by the Programme’s Governance Group (based on discussions with their constituents)
Ischaemic CVD Detection
The number of people enrolled in the PHO who have had an ischaemic CVD event or diagnosis of CVD event as per the CVD definition and have this coded appropriately in the practice PMS system.
CVD Risk Assessment
The number of people enrolled in the PHO that are eligible (as defined) for CVDRA who have had a valid CVD risk recorded within the last 5 years.
CVD Risk Assessment > = 15%
The number of people enrolled in the PHOs who have a CVD risk recorded within the last 5 years and whose CVD risk is >=15% as:
(a) a proportion of the number of eligible patients screened in the five years up to and including the end of the reporting period
(b) a proportion of the expected number of people enrolled in the PHO to have a risk >=15%.
Diabetes Detection
The number of the PHO enrolled population that would be expected to have been diagnosed with diabetes and have this coded appropriately in the practice PMS system.
Diabetes Follow-Up
The number of the PHO enrolled population that have been diagnosed with diabetes that have had a diabetes annual review (Get Checked or equivalent) in the reporting period.
Indicator implementation dates are still to be confirmed.

Please be advised that these shows (scheduled in July and August) have now been postponed until the Programme is able to confirm what the new CVD and Diabetes indicators will be, the reporting requirements associated with the new indicators and implementation timeframes.

A successful first meeting of the PHO Performance Programme Governance Group was held on Friday 30 May. The new group is the result of engagement over several months between PHOs, Providers, DHBs and the Ministry of Health. This produced an agreement to an equal partnership, with four members from PHOs, four members from Providers (nominated by the GPLF), and four members from DHBs (2) DHBNZ (1) and Ministry of Health (1).
Terms of Reference for the PHO Performance Programme Governance Group, and the PHO Performance Programme Advisory Group were agreed.

Nick Chamberlain (Chair of the Programme’s Advisory Group) and Serena Curtis-Lemuelu (Programme Manager) also sit on this group as ex-officio members.
There was healthy discussion on the range of activity under way in the programme. The group also found it useful to discuss how each of the three constituent groups comes to a view on an issue.
The next meeting is scheduled for 25 July after which the Programme expects to be in a position to advise DHBs, PHOs and MSOs of the new CVD and Diabetes indicators, associated requirements and implementation timeframes.

The independent review of Programme services conducted by Monarch Consulting Ltd over the last few months has been completed and a final report provided to DHBs..
It is likely there may be some changes to the way in which the Programme delivers some of its services in the future – all changes will be communicated to Programme contacts.

64 PHOs have had their CVD and Diabetes Capability Assessment Plans approved by their respective DHBs. These plans have a 16% financial weighting attached which is paid as a ‘one off payment’ on the 15th of the month following DHB approval. The financial component is available only until 30 June 2008 after which the plan becomes a Programme prerequisite for future CVD and Diabetes indicators.
Any outstanding approvals must be received by the Programme no later than 30 June 2008 for a payment to be made.

Templates (covering the period 1 Apr to 30 June 2008) have been sent to all DHBs to follow up with their respective PHOs.
All templates are due back to the Programme by 21 July 2008 so the validation processes can be initiated.

All Programme reports for the 1 July to 31 December 2007 period have been rerun and updated on the DHBNZ website. The reports were rerun due to the Programme realising that the change to the Programme Goal for the Cervical Screening indicator had not been implemented. The indicator goal (for both Total Population and High Needs) was originally reported as > 85% and it should have been reported as > 75% to align with that of the National Screening Unit.
Those PHOs affected by the incorrectly reported Programme Goal were contacted and their payments adjusted and paid on 15 June 08.
An opportunity to review targets set for the 1 Jan to 30 Jun 2008 reporting period is being made available to DHBs and PHOs. Targets originally agreed in October 2007 will be adjusted by the Programme according to the PHOs’ achievement for the 1 Jul to 31 Dec 2007 reporting period and forwarded to DHBs to confirm with PHOs.
DHBs will need to confirm targets with the Programme by early July, The Programme will be forwarding the target information to DHBs shortly.
