A single hardcopy of the PHO Performance Programme Annual Report for the period covering 1 July 2009 to 30 June 2010 is being sent to every PHO, DHB and Governance Group member. The report details the activities that have been undertaken by the Programme team during that period and also includes information on national performance against the Programme’s National Performance Indicator Set and a financial summary.

In addition to this Annual Report the Governance Group is seeking your feedback on the PHO Performance Programme against a number of key factors important to a successful quality improvement programme. Your feedback will be used to guide direction and improvement initiatives.
The formal feedback process initiated in this survey will provide structured stakeholder feedback and higher statistical validity than the self-evaluation completed by the team and included in the Annual Report.
