The formula for calculating the CVD and Diabetes prevalence estimates for the Programme has been developed by the Ministry of Health and the University of Auckland. Prevalence estimates for each PHO and their practices have been forwarded to the Programme to review.
We will be providing each PHO with a value that indicates the percentage of people within their enrolled population who, in the case of the:
1. Ischaemic CVD Detection indicator – are expected to be coded
2. Diabetes Detection indicator – are expected to be coded
3. Diabetes Follow Up After Detection indicator – are expected to have had a diabetes annual review after they have been coded with diabetes
The Programme expects to be in a position to forward prevalence information to you within the next week.
Note prevalence estimates do not apply to the CVD Risk Assessment indicator as this denominator is the number of patients coded.

The Programme is currently updating the Indicator Definitions for PHOs document (version 2) to include how the data is extracted from PMS systems to PHOs and national data collections. The document will also be updated to include how the data is then forwarded to or extracted by the Programme to measure the performance indicators.
The Programme expects to have the updated document (version 3) available on the DHBNZ website within the next week. A communication confirming the availability of the document will be sent to all Programme DHB, PHO and Ministry contacts.

The Programme is currently developing a process to formalise the means in which changes can be proposed to any component of the PHO Performance Programme. With interest in the Programme increasing, the Programme recognises that a robust method for identifying, documenting, developing, approving and implementing changes to the Programme is required.

MartinJenkins has completed their evaluation of the Programme and a final report is expected at the end of November. Letters of thanks are being sent to all informants who participated in the evaluation.

The final Advisory Group meeting with the current membership was held on 14 October. 13 out of 23 members were able to attend. Key topics discussed were:
The next Advisory Group meeting consisting of the new membership will be held next year on a date to be scheduled
Applications to be a member of the Programme’s Advisory Group closed on the 20th October. The Programme received applications from people from a number of different organisations and with a variety of skill sets.
The Programme’s Governance Group considered the applications at their meeting held on 14th November. Applicants will be advised shortly and an announcement will be made following that.

The PHO Performance Programme Governance Group held another successful meeting last Friday 14 November in Wellington.
Strategic Direction
A set of Strategic Principles for the Programme was developed for discussion by Governance Group members with their PHO, Provider and DHB networks before the next meeting in February 2009.
Data Security
Data security and access came through as a key issue in the strategy discussion. Further work is being done to clearly describe all data flows in the programme and the security arrangements around those flows. The programme does not provide patient-identifiable information, and there is a need to describe all flows so that the sector has full confidence in the Programme's activities. There will be a significant discussion on this at the next Governance Group meeting.
Public Reporting
While wider data security is being clarified, there is unease about Public Reporting. Group members agreed to seek comment from their constituents on whether DHB level reporting would be acceptable. Any other proposals would be on hold until wider data security is clarified.
Indicator Evaluation Process
A paper from the Advisory Group was received. The paper outlined the criteria to be taken into account for development of new indicators and evaluation of indicator performance. The Advisory Group will do further work to add components including PHO and Provider capacity and capability to deliver against proposed indicators.
CVD and Diabetes Weightings and Targets
CVD and Diabetes Indicator weightings and targets were agreed. The indicators themselves were agreed last meeting and apply from 1 July 2008. The PSAAP process will be will be advised of the decision and asked to implement the decision into the PHO Contract.
Advisory Group
New members were selected for the Advisory Group following a nominations process. The Advisory Group provides practical guidance, advice and expertise to the programme and is the key group for dynamic debate and testing of Programme content, clinical relevance and business sustainability in order to deliver on the Programme's quality improvement purpose. The new members will be advised shortly, and the first meeting of the refreshed group will occur in March 2009

Please be advised that the process for receiving enquiries related to the Programme is now being managed centrally. This enables the Programme to be able to log enquiries and track progress so that responses can be prepared and disseminated in a timely and consistent manner.

Programme performance reports and scorecards (covering the performance period 1 January to 30 June 2008) have been posted to the DHBNZ website.

Given the tight timeframe in which PHOs and DHBs were required to verify the details of the reports with the Programme, all payments relating to the reports were made to PHOs on 17th November 2008. DHBs and PHOs have until 21st November to raise any issues regarding the reports with the Programme and where further investigation reveals that changes to the reports are necessary, then the Programme will update the reports accordingly and perform a ‘wash up’ of payments for those PHOs that are affected. Wash up payments will be processed on 15th December 2008.
Flat file data pertaining to the Programme reports has also been forwarded to PHOs.

Please note that the Programme is aware that the Ministry recently updated their NZDep2001 classifications to NZDep2006. As a result of the update, some CBF registers validated by the Ministry of Health as part of the CBF register process may have incorrectly coded PHO enrolees under ‘Quintile ‘0’, which has therefore impacted some PHOs performance this quarter due to the change in High Need and Total population figures. The Programme has raised this issue with the Ministry and will continue to follow its progress. The Programme firmly believes that any data issues should be addressed and corrected at data source..
